Medication

Students who require medication (over the counter and prescription) in school must have a written order from their health care provider. Please submit all medications and physician's orders directly to the school nurse.  All medication order forms must be signed and stamped by the medical provider.  Main office staff are not able to accept medications.

• Asthma Treatment Plan (for students who require an inhaler or nebulizer during school hours)
• Seizure Action Plan
• Authorization for Emergency Medicine (for students who require epinephrine for life threatening allergic emergencies)
• Medication Administration Request Form  (This form should be used for medication that needs to be administered during the school day (ex. ADHD medication, over the counter pain relievers, etc.)
