Registration

A physical exam is required for all students entering the Somerset Hills School District Public for the first time (Preschool, Kindergarten and students who transfer into the district). The Universal Child Health Record along with a copy of the most recent Immunizations must be submitted prior to entry.  Please note that any physical exam that was performed by a health care provider within 365 days prior to the first day of school will be acceptable.

Physical Exam Form for New Students: The New Jersey Department of Education recommends that students obtain a complete physical examination at least once during each developmental stage: early childhood (Pre-K - Grade 3), pre-adolescent (Grade 4 - Grade 6), and adolescent (Grade 7 - Grade 12).
