Required Documents
In order to rent our facilities, the following forms must be submitted:

1. Security Training for Facility Users Acknowledgment Form
   Drill Review  Drill Review Acknowledgement Form
2. Hold Harmless Form
   HH form
3. 2431.4 Policy Acknowledgment Form (if applicable)
   Policy 2431.4  Policy 2431.4 Acknowledgement Form

In addition to these forms, a valid Certificate of Insurance must also be submitted.
